Dear Sir/Madam,

1. I, natural/legal guardian of my child along with the father/mother/legal guardian of our child confirm that we are above the age of majority as per applicable law in India and hereby expressly consent, permit and state that I have no objection whatsoever for my child/ward participating in the auditions of the program titled "Dance+ Pro" ("Program") and submitting/sharing my/his/her personal information, contact details and the video of his/her dance performance/clip to Novi Digital Entertainment Private Limited ("Novi") in the manner prescribed and also to Frames Production Company Private Limited ("Production House") who is producing the Program for exploitation on Disney+ Hotstar platform, or any other channel/digital platform of Novi or its affiliates ("Platforms and Channels"). I confirm and agree that I have read the Terms and Conditions applicable to the Program uploaded at https://secure-media.hotstar.com/web-assets/prod/tnc/danceplus_tnc.html ("Terms and Conditions") and irrevocably and unconditionally agree, on behalf of my child to adhere to the provisions of such Terms and Conditions.

2. We, jointly and severally agree and undertake that in the event our child is shortlisted to participate in further rounds of the Auditions of the Program, we shall, at all times accompany him/her for the Auditions and the shoot of the Program, and sign and execute all the necessary legal documents on behalf of us and our child as may be required by Novi and/or the Production House, at their sole discretion to protect or enforce any of the rights granted or confirmed by our child in connection to the Program and/or any services to be rendered by our child towards the Program and/or Novi and/or the Production House. We shall be wholly responsible for the well- being of our child.

3. We jointly and severally agree to provide any additional document as required from time to time by Novi and/or the Production House to substantiate our relationship with our child. Further the information and documents submitted herein are true to our knowledge and record. We, jointly and severally agree to comply with the provisions of any documentation that may be signed and submitted by us along with the provisions of the Terms and Conditions in respect of our child’s participation in the Program.

I as the natural/legal guardian of my child state that I have read and understood the foregoing statement, and am competent to execute this agreement on behalf of my child and have the full right and authority to execute this letter, give consent and to grant the authority for exploitation of all rights as stated hereinbefore.
